      <image:title>Projects (old) - Michigan Alliance for Innovation on Maternal Health (MI AIM)</image:title>
      <image:caption>In fiscal year 2021, SEMPQIC began formally supporting MI AIM in Region 10 with funding from the Michigan Department of Health and Human Services (MDHHS). The goal of MI AIM is “to eliminate preventable maternal mortality, severe maternal morbidity, and disparity.” This goal is achieved through the implementation of specific  clinical protocols which are then reported on by the birthing hospitals. The clinical protocols include Severe Hypertension, Obstetric Hemorrhage, Sepsis, and Reduction of Peripartum Racial/Ethnic Disparities. SEMPQIC has partnered with MI AIM to engage the birthing hospitals in Region 10 in adopting these protocols Bundles. Our efforts focus on encouraging the the 23 birthing hospitals in Region 10 with the specific goal of improving participation for structural and process variables. These eight hospitals provide data, including trends, to MI AIM and our hospital dashboard. The work began by meeting with each of the hospitals, which resulted in developing crosswalks between existing quality efforts within the hospital and MI AIM Bundle processes. Through this, we were able to improve data submission.</image:caption>

      <image:title>Projects (old) - Healthy Baby at Home (HB@H)</image:title>
      <image:caption>SEMPQIC Healthy Baby at Home (HB@H) quality improvement project began in 2017. HB@H serves as a process improvement project to increase home visiting participation for high-risk pregnant women and for babies cared for in an intensive care unit (NICU). These efforts use an evidence-based approach and support existing evidence- based strategies in a meaningful way, while acknowledging social determinants of health and the impacts of inequities in the healthcare delivery system. The service model consists of providers partnering with Maternal Infant Health Programs (MIHP), and other home visiting agencies, to provide both direct referral and access to their client population for case finding efforts. The agencies are situation on-site to engage clients face-to-face during the referral process, with a mechanism to support communication around referrals being established. Referrals were encouraged for all discharges from the NICU. MIHP referral could occur in conjunction with a skilled nursing home referral. The communication process established referral and feedback processes with providers in the prenatal clinic. Case finding occurred while MIHP agencies were available within clinics to approach potential clients.</image:caption>

      <image:title>Projects (old) - Detroit Health Equity Education Resource: D-HEER</image:title>
      <image:caption>The W.K. Kellogg Foundation (WKKF) awarded SEMPQIC a $450,000 three-year grant (as $150,000 per year) to create the Detroit Health Equity Network, beginning October 2020. The Detroit Health Equity Education Resource (D-HEER) is being developed using a collective impact approach enveloping the “Water of Systems Change,” framework as described by FSG. The intent of using this framework is to shift the conditions that hold disparate birth outcomes in place in Detroit. D-HEER will be an online platform for health equity information, resources, and best practice exchange that will assist organizations to better serve those at high-risk for experiencing maternal and infant mortality. The site will also create community connections that foster capacity growth for agencies to implement equitable policies, practices, and programs that create system change for improved birth and maternal health outcomes. The platform will expand to enable the dissemination of lessons learned statewide and will coordinate efforts with MDHHS in the promotion of the Mother Infant Health and Equity Improvement Plan. In late 2020, 46 individuals from diverse backgrounds, such as published authors, university leadership, health insurance providers, birthing hospital personnel, federally qualified health centers, medical students, home visitors, breastfeeding proponents, community activists, and faith-based and community-based organizations were invited to participate as members of a Collective Impact Advisory Committee. This committee was tasked with helping to shape the direction of the Network and members were placed into three work groups, each with a specific task in coordinating the Network. The first group worked to identify resource materials and establish criteria for resource selection in the Network. The second group defined the methods for sharing use of the resources with end-users and the community. The third group collected data elements for inclusion in the data dashboard and ensured the evaluation plan for the Network appropriately captures metrics to reflect the impact of this effort. Advisory Committee members actively participate in the D-HEER site, and provided input before the site was made available to the public.</image:caption>

      <image:title>Projects (old) - Project Detroit: Voices for Life</image:title>
      <image:caption>Project Detroit: Voices for Life was established to build on existing community assets to examine and replicate circumstances and conditions where Black mothers thrive, empowering Black women to advocate for the best perinatal care experiences, and lead care providers to reach their full potential in providing respectful and equitable care for Detroit women. Project Detroit: Voices for Life was a major addition to SEMPQIC’s portfolio in 2021. It was funded as part of the second cohort under Merck for Mother’s Safer Childbirth Cities initiative. SEMPQIC received $1 million over three years to support this project, which will impact Black maternal mortality in the city of Detroit. Work on the project began in January 2021, with work in its current fiscal year ongoing. Program partners include: The Detroit Health Department, which is focused on establishing a Maternal Mortality and Vitality  Review Team to analyze the contributing factors to maternal mortality. This initiative uses an innovative approach to examine maternal deaths and survival aspects at a local level in order to identify potential system enhancements that may reduce the persistent racial disparities in health outcomes in Detroit. Additionally, the Detroit Health Department is enhancing the work of the existing SisterFriends program to recruit “Storytellers” to share their birthing experiences, to change the perinatal narrative for Black and Brown women. Black Mothers Breastfeeding Association (BMBFA), which is focusing on systems change by working with stakeholders to integrate and sustain community-based doulas as a part of the health care system. Henry Ford Health System is continuing its work on addressing structural unconscious bias in health care providers by expanding their developed training in collaboration with the March of Dimes. Focus: HOPE is collaborating with BMBFA and SisterFriends to recruit Storytellers, as well as working with men and fathers to educate them on the roles they can play in improving maternal health outcomes</image:caption>

      <image:caption>Alethia Carr RD, MBA Alethia Carr, is a native Detroiter, who retired as a public health administrator, and former Director of Michigan’s Maternal and Child Health Administration at MDHHS. Alethia is the Health Equity lead consultant to SEMPQIC, and works to improve the perinatal health system, through improvement of equity in health services and birthing outcomes. Ms Carr has worked as a public health consultant to a variety of states and national organizations since her full time retirement in 2013, with a focus on addressing health equity in perinatal care by recognizing the impact of the social determinants that impact health. Alethia believes all people are entitled to optimal health. Ms. Carr serves on several boards of directors, including: Kids Health Connection, a community based agency providing health support services to children and families in the tri-county area; and Oak Village C.A.R.E.S. , a community development corporation in Detroit. Additionally, she is an active, long time member of Oak Grove A.M.E. Church. Alethia has a Master’s degree in Business Administration, is a Registered Dietitian and a former Food and Policy Fellow of the W.K. Kellogg Foundation. She is a wife, and mother to two adult children. She has lived in Detroit, Michigan all her life, and traveled to nearly 40 states, many countries, and several continents.</image:caption>

      <image:caption>Dr. Iris Taylor RN, Ph.D. A lifelong resident of Detroit and graduate of Detroit public schools and universities, Iris Taylor has dedicated more than 45 years to improving the health and welfare of the citizens of Detroit through her professional career and volunteer service. Dr. Taylor is an achiever in every sense of the word. She received her bachelor and master’s degrees in nursing and her Doctor of Philosophy from Wayne State University. She also completed her management fellowship at the Wharton School of Business. Her career has included a variety of positions including that of practitioner, educator/consultant and hospital president/CEO. Dr. Taylor has served two terms as President and as CEO of Detroit Receiving Hospital and University Health Center. She served as Chief Business Officer for the Detroit Medical Center and was president of Harper University Hospital and Hutzel Women’s Hospital. Taylor left her position as President and CEO of the Detroit Institute for Children to become the first Detroit Medical Center’s Chief Nursing Officer. Dr. Taylor is an adjunct professor at Wayne State University in two prestigious colleges/schools--College of Nursing and School of Medicine. She is very dedicated to the community and has served in leadership positions on many local boards of directors. She served as president of Detroit Public School Community District’s Board as well as the vice chair of the Detroit Wayne Integrated Health Network ( Detroit Wayne Mental Health Authority).  Dr. Taylor has been honored numerous occasions for her leadership and contribution to the community.</image:caption>

      <image:title>SEMPQIC Events - Reading Circle</image:title>
      <image:caption>SEMPQIC started the Reading Circle in 2020 with its internal team as a way of learning about issues related to health equity together. A facilitated discussion of the New York Times Magazine, 1619 was held, using 90 minutes of discussion about the material and its implications for and about health equity. In 2021, an invitation to participate in the Reading Circle was extended to SEMPQIC membership at large. Several sections of the Roots of Health Inequity, an on-line curriculum, was read and discussed over several months. This curriculum was developed by the National Association of City and County Health Officials and offered history, a case study and stimulating material that produced insight into the current public health inequities across various systems. The Reading Circle is meant to increase the knowledge capacity of the SEMPQIC leadership team and its membership about issues that impact and influence the current system of perinatal care and that continue to support the unacceptable disparate health outcomes of Black and Brown people. In 2022, SEMPQIC Reading Circle read The Burden-African Americans and the Enduring Impact of Slavery. The author Rochelle Riley led the facilitated discussion.       <image:title>Michigan Alliance for Innovation on Maternal Health (MI AIM) - In fiscal year 2021, SEMPQIC began formally supporting MI AIM in Region 10 with funding from the Michigan Department of Health and Human Services (MDHHS)</image:title>
      <image:caption>SEMPQIC works with the Michigan Alliance for Innovation on Maternal Health (MI-AIM) and  local birthing hospitals to implement nationally recognized patient safety bundles aimed at improving maternal health outcomes. These bundles focus on critical areas such as severe hypertension, obstetric hemorrhage, sepsis, and reducing racial and ethnic disparities in maternal care. Hospitals that adopt these protocols regularly report on their progress to ensure accountability and drive continuous improvement. With a $55,000 grant from SEMPQIC, Trinity Health Oakland, Huron Valley-Sinai Hospital and Corewell Health Trenton each implemented targeted initiatives to improve maternal health outcomes and patient safety. Trinity Health Oakland focused on clinical innovation and equity, Huron Valley-Sinai Hospital prioritized patient safety and staff development, and maternal sepsis training, and Corewell Health Trenton addressed care access gaps by launching an educational campaign on postpartum complications.</image:caption>

      <image:title>Healthy Baby at Home - SEMPQIC Healthy Baby at Home (HB@H) quality improvement project began in 2017.</image:title>
      <image:caption>HB@H serves as a process improvement project to increase home visiting participation for high-risk pregnant women and for babies cared for in an intensive care unit (NICU). These efforts use an evidence-based approach and support existing evidence- based strategies in a meaningful way, while acknowledging social determinants of health and the impacts of inequities in the healthcare delivery system. The service model consists of providers partnering with Maternal Infant Health Programs (MIHP), and other home visiting agencies, to provide both direct referral and access to their client population for case finding efforts. The agencies are situation on-site to engage clients face-to-face during the referral process, with a mechanism to support communication around referrals being established. Referrals were encouraged for all discharges from the NICU. MIHP referral could occur in conjunction with a skilled nursing home referral. The communication process established referral and feedback processes with providers in the prenatal clinic. Case finding occurred while MIHP agencies were available within clinics to approach potential clients.</image:caption>

      <image:title>Detroit Health Equity Education Resource: D-HEER - A SEMPQIC Project.</image:title>
      <image:caption>The W.K. Kellogg Foundation (WKKF) awarded SEMPQIC a $450,000 three-year grant (as $150,000 per year) to create the Detroit Health Equity Network, beginning October 2020. The Detroit Health Equity Education Resource (D-HEER) is being developed using a collective impact approach enveloping the “Water of Systems Change,” framework as described by FSG. The intent of using this framework is to shift the conditions that hold disparate birth outcomes in place in Detroit. D-HEER will be an online platform for health equity information, resources, and best practice exchange that will assist organizations to better serve those at high-risk for experiencing maternal and infant mortality. The site will also create community connections that foster capacity growth for agencies to implement equitable policies, practices, and programs that create system change for improved birth and maternal health outcomes. The platform will expand to enable the dissemination of lessons learned statewide and will coordinate efforts with MDHHS in the promotion of the Mother Infant Health and Equity Improvement Plan. In late 2020, 46 individuals from diverse backgrounds, such as published authors, university leadership, health insurance providers, birthing hospital personnel, federally qualified health centers, medical students, home visitors, breastfeeding proponents, community activists, and faith-based and community-based organizations were invited to participate as members of a Collective Impact Advisory Committee. This committee was tasked with helping to shape the direction of the Network and members were placed into three work groups, each with a specific task in coordinating the Network. The first group worked to identify resource materials and establish criteria for resource selection in the Network. The second group defined the methods for sharing use of the resources with end-users and the community. The third group collected data elements for inclusion in the data dashboard and ensured the evaluation plan for the Network appropriately captures metrics to reflect the impact of this effort. Advisory Committee members actively participate in the DHEER site, and provided input before the site was made available to the public.</image:caption>

      <image:title>Maternal Health - In fiscal year 2021, SEMPQIC began formally supporting MI AIM in Region 10 with funding from the Michigan Department of Health and Human Services (MDHHS)</image:title>
      <image:caption>The goal of MI AIM is “to eliminate preventable maternal mortality, severe maternal morbidity, and disparity.” This goal is achieved through the implementation of specific  clinical protocols which are then reported on by the birthing hospitals. The clinical protocols include Severe Hypertension, Obstetric Hemorrhage, Sepsis, and Reduction of Peripartum Racial/Ethnic Disparities. SEMPQIC has partnered with MI AIM to engage the birthing hospitals in Region 10 in adopting these protocols Bundles. Our efforts focus on encouraging the the 23 birthing hospitals in Region 10 with the specific goal of improving participation for structural and process variables. These eight hospitals provide data, including trends, to MI AIM and our hospital dashboard. The work began by meeting with each of the hospitals, which resulted in developing crosswalks between existing quality efforts within the hospital and MI AIM Bundle processes. Through this, we were able to improve data submission.</image:caption>

      <image:caption>Southeast Michigan Perinatal Quality Improvement Coalition (SEMPQIC) was launched in 2015 with funding from The Michigan Department of Health and Human Services to implement the 2015 Mother Infant Health Improvement Plan, updated for 2020-2023 as the Mother Infant Health &amp; Equity Improvement Plan in Region 10, Wayne, Oakland and Macomb Counties. We are now working on the Advancing Healthy Births: An Equity Plan for Michigan Families and Communities plan as part of the October 2023 version of MDHHS strategic plan for improving birth outcomes.</image:caption>

      <image:title>Fatherhood - SEMPQIC Fatherhood Initiative and Curriculum</image:title>
      <image:caption>SEMPQIC, in taking up the charge to address and reduce the significant disparities in maternal health outcomes experienced by Black mothers, understands the critical need to recognize and include the role of fathers/father figures. Engaging fathers in maternal and perinatal care not only supports mothers but also promotes better health outcomes for families. We know that Black fathers are an essential influence to building strong children, leading healthy families, and stabilizing communities. SEMPQIC and our Black Fatherhood project leads have designed a community outreach program that focuses on educating fathers about the benefits of engaged fatherhood. The Color of Fatherhood Curriculum This guide was designed to support Black dads in the perinatal journey. The program will attempt to address the historical factors preventing Black fathers from fully participating in their families and provide them with the support to elevate their resiliency and dedication in the lives of their families. The course includes tools, knowledge, and community to navigate challenges and embrace their roles effectively.</image:caption>

      <image:caption>SEMPQIC started the Reading Circle in 2020 with its internal team as a way of learning about issues related to health equity together. A facilitated discussion of the New York Times Magazine, 1619 was held, using 90 minutes of discussion about the material and its implications for and about health equity. In 2021, an invitation to participate in the Reading Circle was extended to SEMPQIC membership at large. Several sections of the Roots of Health Inequity, an on-line curriculum, was read and discussed over several months. This curriculum was developed by the National Association of City and County Health Officials and offered history, a case study and stimulating material that produced insight into the current public health inequities across various systems. The Reading Circle is meant to increase the knowledge capacity of the SEMPQIC leadership team and its membership about issues that impact and influence the current system of perinatal care and that continue to support the unacceptable disparate health outcomes of Black and Brown people. In 2022, SEMPQIC Reading Circle read The Burden-African Americans and the Enduring Impact of Slavery. The author Rochelle Riley led the facilitated discussion.       <image:caption>We are leading a new culture of collaboration and coordination among the myriad of health systems, primary care and obstetrical providers, emergency services, home visiting programs, and support services to ensure that mothers and infants receive optimal care before, during and after pregnancy. SEMPQIC is developing strategies that address the life-course and social determinants of health in the effort to reduce infant mortality and as support for the perinatal system of care.       <image:caption>The SEMPQIC team uses a health equity lens in every aspect of our perinatal work. In the 2021 fiscal year, SEMPQIC began work on two health equity-focused projects, one funded by the W.K. Kellogg Foundation and the other by Merck for Mothers. Both initiatives are focused on improving the health equity of Black and Brown populations in the City of Detroit, across Region 10 and the state of Michigan. As a central focus of our work, SEMPQIC regularly discusses health equity topics during our coalition meetings. The intent is for every coalition meeting to feature an interactive session highlighting an area of equity within the healthcare delivery system. We do this to foster a dialogue between coalition members and contribute to individual and organizational transformation.</image:caption>

      <image:caption>Southeast Michigan Perinatal Quality Improvement Coalition (SEMPQIC) was launched in 2015 with funding from The Michigan Department of Health and Human Services to implement the 2015 Mother Infant Health Improvement Plan, updated for 2020-2023 as the Mother Infant Health &amp; Equity Improvement Plan in Region 10, Wayne, Oakland and Macomb Counties. We are now working on the Advancing Healthy Births: An Equity Plan for Michigan Families and Communities plan as part of the October 2023 version of MDHHS strategic plan for improving birth outcomes.</image:caption>

      <image:caption>Dr. Renée Branch Canady serves as Chief Executive Officer (CEO) of MPHI, a public health institute comprised of process and subject matter experts with a shared vision of a just world that is healthier for all. Dr. Canady is a relationship driven leader and author of the Amazon best seller book, “Room at the Table: A Leader’s Guide to Advancing Health Equity and Justice.” In this well-received book, Canady presents a leadership challenge to everyone seeking to understand the intricacies of equity and justice, pushing them to become change agents within health/public health and other fields. Prior to joining MPHI, Dr. Canady served as Health Officer and Director of the Ingham County Health Department, located in Lansing, Michigan. In that role she oversaw the county’s statutory responsibility to protect and promote the health of county residents and lead the development of the innovative and nationally known, Ingham County Health Equity Social Justice program. Dr. Canady has held faculty and administrative positions at Michigan State University in the College of Nursing where she developed a research trajectory in maternal health and health inequities. She was an assistant professor in the C.S. Mott Department of Public Health, where she developed and chaired the Master of Public Health (MPH) core course “Health Equity for Public Health Practitioners.” Dr. Canady is recognized as a national thought leader in the areas of health inequities and disparities, cultural competence, and social justice. She has published and presented broadly on these topics and her passion for this work is evident in her personal, academic, and professional life. While serving on numerous national boards, review panels, and advisory groups, Dr. Canady has been highly influential in broadening the discussion of working across difference for the health of all. Canady earned her PhD in Medical Sociology from Michigan State University, a master’s degree in public administration from Western Michigan University and bachelor’s degree in public health nutrition from the University of North Carolina at Chapel Hill.</image:caption>

      <image:caption>Theodore Jones, M.D., F.A.C.O.G., is Director of Maternal Fetal Medicine at Corewell Health Dearborn. Dr. Jones is also a member of the Division of Maternal Fetal Medicine and the Division of Fetal Imaging in the Department of Obstetrics and Gynecology at Beaumont Hospital Royal Oak.  Dr. Jones is a graduate of Morehouse College in Atlanta GA. He received his medical degree from Temple University School of Medicine in Philadelphia. He completed his Obstetrics and Gynecology residency at Baylor University Medical Center in Dallas TX. He was a National Health Service Corp scholar and served three years in rural Arkansas (Pine Bluff) to satisfy his commitment. He completed his fellowship in Maternal Fetal Medicine at Wayne State University/Hutzel Hospital. He was a faculty member at Wayne State University School of Medicine, Department of Obstetrics and Gynecology from 1991 until his departure in 2017. While there, he had multiple administrative roles in the department over two decades, including serving twice as residency program director, division chief of Maternal Fetal Medicine, associate chair for Education, and as interim chair. He also has been chief of Obstetrics for Hutzel Women’s Hospital. He is a fellow of the American College of Obstetricians and Gynecologists and is a Diplomate of the American Board of Obstetrics and Gynecology for general Obstetrics and Gynecology as well as Maternal Fetal Medicine, for which he served as a subspecialty board examiner for 18 years.</image:caption>

      <image:caption>Dr. Steven Ondersma is C.S. Mott Endowed Professor of Public Health in the Charles Stewart Mott Department of Public Health at Michigan State University, and Director of the e-Health in Pregnancy (e-HELP) Lab. His primary interest is in population-level interventions promoting maternal and child health in underserved communities, particularly via technology-based brief interventions in the perinatal period. He is a Fellow of the American Psychological Association (Division 37), former Editor of the journal Child Maltreatment, a member of the Motivational Interviewing Network of Trainers, and currently serves on the Board of Scientific Counselors for the CDC’s National Center for Injury Prevention and Control. He previously co-chaired the Neonatal Opioid Withdrawal Syndrome subgroup of the NIH’s Environmental Influences on Child Health Outcomes (ECHO) initiative. He has led multiple NIH and CDC-funded studies focusing on the development, validation, and implementation of novel screening techniques and digital interventions in healthcare settings. Dr. Ondersma is also leading development of the Computerized Intervention Authoring System (CIAS 3.0), an NIH-funded, non-commercial, open research resource facilitating development of interactive and sophisticated digital interventions for any platform without coding (available at www.cias.app). Applied uses of CIAS 3.0 include community-directed direct development of digital interventions through hackathon-style “Appathons” and a program integrating digital screening, feedback, and facilitation of service receipt into prenatal care clinics statewide (www.ht-2.org).</image:caption>

Beyond her doula work, she is an advocate and educator for maternal health, speaking at events and leading workshops to educate and empower women.       <image:caption>Reginald Day is a Certified Lactation Counselor and Community Health Worker with over 15 years of experience serving families and specifically fathers. He is also Mental Health First Aid trained. Specializing in fatherhood and perinatal support, Reginald uses his experience to walk alongside families and fathers during the parenthood journey by supporting with coaching, community resources and connections.   By ensuring that the whole family is centered and that the SDoH needs are factored into the care of families, Reginald has made good use of his bachelor’s degree in Health Service Administration from University of Detroit Mercy along with his CLC and CHW certification. Having served as an administrator in home health for over 12 years he decided to follow his passion for serving and uplifting families. Over the years, Reginald has grown his community connections and shared his experience by participating in many local, state, and national conferences, ensuring that fathers are centered along with mom and baby in maternal and infant health care.       <image:caption>Alethia Carr, is a native Detroiter, who retired as a public health administrator, and former Director of Michigan’s Maternal and Child Health Administration at MDHHS. Alethia is the Health Equity lead consultant to SEMPQIC, and works to improve the perinatal health system, through improvement of equity in health services and birthing outcomes. Alethia has worked as a public health consultant to a variety of states and national organizations since her full time retirement in 2013, with a focus on addressing health equity in perinatal care by recognizing the impact of the social determinants that impact health. Alethia believes all people are entitled to optimal health. Alethia serves on several boards of directors, including: Kids Health Connection, a community based agency providing health support services to children and families in the tri-county area; and Oak Village C.A.R.E.S. , a community development corporation in Detroit. Additionally, she is an active, long time member of Oak Grove A.M.E. Church. Alethia has a Master’s degree in Business Administration, is a Registered Dietitian and a former Food and Policy Fellow of the W.K. Kellogg Foundation. She is a wife, and mother to two adult children. She has lived in Detroit, Michigan all her life, and traveled to nearly 40 states, many countries, and several continents.</image:caption>

      <image:caption>The SEMPQIC team uses a health equity lens in every aspect of our perinatal work. In the 2021 fiscal year, SEMPQIC began work on two health equity-focused projects, one funded by the W.K. Kellogg Foundation and the other by Merck for Mothers. Both initiatives are focused on improving the health equity of Black and Brown populations in the City of Detroit, across Region 10 and the state of Michigan. As a central focus of our work, SEMPQIC regularly discusses health equity topics during our coalition meetings. The intent is for every coalition meeting to feature an interactive session highlighting an area of equity within the healthcare delivery system. We do this to foster a dialogue between coalition members and contribute to individual and organizational transformation.</image:caption>
